What BMI Does — and Doesn't — Tell You
Where the BMI formula came from, what the standard categories mean, and the well-documented cases where the number misleads.

Body Mass Index is one of the most used — and most argued-about — numbers in health. It's worth understanding both what it is and what it was ever meant to do, because most of the arguments come from asking a screening statistic to be a diagnosis.
The formula and where it came from
BMI = weight (kg) ÷ height (m)² — or in US units, 703 × weight (lb) ÷ height (in)². The idea dates to the 1830s, when the Belgian statistician Adolphe Quetelet was looking for a simple way to describe body weight across a population, normalized for height. That origin matters: BMI was designed to describe groups, and it was later adopted by public-health bodies because it needs nothing but a scale and a tape measure — cheap, repeatable, and comparable across millions of people.
The standard adult categories
For adults 20 and older, the CDC's categories are: under 18.5 underweight; 18.5 to under 25 healthy weight; 25 to under 30 overweight; and 30 or above obesity, which is further divided into Class 1 (30–35), Class 2 (35–40), and Class 3 (40+). To make that concrete with the CDC's own example: for an adult who is 5'9", the healthy range spans 125–168 pounds, overweight begins at 169, and obesity at 203. At the population level these thresholds correlate with risk for conditions like type 2 diabetes and cardiovascular disease — which is exactly the job a screening measure is supposed to do.
Where the number misleads
BMI cannot tell muscle from fat, and muscle is denser. Muscular athletes routinely read "overweight" at low body-fat percentages — a 5'9" athlete at 175 lean pounds lands at BMI 25.8. It also says nothing about where fat is carried, even though abdominal (visceral) fat carries more metabolic risk than fat elsewhere; two people with identical BMIs can have very different risk profiles. The standard cutoffs fit some populations better than others — several health bodies use lower risk thresholds for adults of Asian descent, for example — and BMI shifts meaning with age as muscle mass declines. It isn't interpreted the same way at all for children and teens (who use age-and-sex percentiles) or during pregnancy. The CDC itself is direct about this: BMI is a screening measure, one indicator to be considered alongside others, not a diagnosis.
How to use it sensibly
Treat BMI as a rough first-pass number that costs nothing to compute. If it flags something, the follow-ups that add real information are things like waist circumference, blood pressure, blood glucose and lipid panels, and an honest look at fitness — measures a clinician can put in context. And if your BMI reads high while you're visibly muscular and metabolically healthy, the number isn't telling you something is wrong; it's telling you it can't see composition.
